During our first conversation (telephone), after having decided to make an appointment, you will be given a code. With this code, you will have access to the forms to be completed prior to our first meeting. You should read and complete all documents that will be provided to you prior to the first meeting - this way we can fully take advantage of the 50 minutes during our first meeting.

If you do not have access to Internet or a printer, it is suggested that you arrive at least 15 minutes in advance of the first meeting, in order to complete and read these forms. When you arrive in the waiting room, the forms will be on a blue clip board on the table in front of the chairs. There is no name and identification on forms at that time, in order to ensure confidentiality.

  • Parental Consent
  • Welcome Interview (18 and under)

    Please note: For children who are 14 years of age or less, the parents or legal guardians must sign the "parental authorization" form.
